EPHRATA BOROUGH, PA -- Shortly before Midnight on Friday July 3rd, BOX Alarm 15 was transmitted to 1166
South State Street at the Goodwill store for the reported building on fire. Units from the Pioneer Fire
Company along with EMS from 112 filled the first alarm with multiple reports of a working fire coming into
Communications. Chief 15 requested two add'l Engine Companies be added to the assignment as Police
Department arrived confirming the reports. Chief 15 arrived on location moments later with heavy fire
showing from the Bravo/Charlie side of the building and ordered Engine 15-1 and Engine 12-1 go heavy duty
with their heaviest lines and wagon pipes as the fire aggressively spread into the building. Crews work for
about 1.5 hours to control the blaze as many hot spots were found through-out the building. The fire in under
investigation and no injuries were reported.

AVON, NY -- On Saturday afternoon, July 4, three people were injured, one seriously, in a head-on crash on
routes 5 & 20, just east of Interstate 390. The East Avon Fire Department and Avon EMS/ALS were dispatched
to the scene. Responding units found an SUV on it's roof and a heavily damaged car in the roadway.
Additional ALS units from Livonia and Henrietta as well as Mercy Flight Central were requested to the scene.
The Village of Avon Fire Department was called to to set up a landing zone for the helicopter on the on the
bridge over I-390. Firefighters worked for over 45 minutes to extricate the female passenger in the car from
the wreckage. She was flown to Strong Memorial Hospital in Rochester with serious injuries. The drivers of
the vehicles were also injured. Just prior to the accident, Livingston County 911 had received several calls
of a vehicle matching the description of the SUV involved in the crash driving erratically. Officials from the
New York State Police and the Livingston County Sheriff's Office responded to the scene and continue to

CONESUS LAKE, NY -- On Saturday afternoon, July 4, Conesus Fire Department and Livonia EMS were
dispatched to a report of a capsized sailboat on the east side of Conesus Lake. Emergency responders
found the sailboat upside down, with two persons in life jackets in the water. The Livingston County Sheriff's
Office Marine Patrol assisted firefighters as they were brought to shore and checked by EMS personnel.

WRIGHTSVILLE BOROUGH, PA -- As members were enjoying crabs at the firehouse, neighbors rushed into the
firehouse yelling that something was burning on Hellam Street. Heavy smoke was visible from the 41 House.
At 20:58 York County 911 dispatched the first alarm assignment and sent Engine 41 into action with a crew of
7 in one minute. Engine 41 grabbed a hydrant infront of Station 41 and laid into the scene finding a working
fire on the 2nd Division with fire blowing out the front and rear windows. Crews from the Engine began
advancing two lines into the fire building. The original Engine crew vented, searched and knocked 90% of
the fire down within 10 minutes. Rescue 41-1 went enroute with an additional 10 personnel and laid another
section of supply line from 3rd and Hellam into the scene. The rescue crew took the exposure building to
check for extension. All that was found was a light smoke condition and high temperatures. All mutual aid
departments assisted in extensive overhaul of the structure. Operations were wrapped up in just over 3

BROOKE COUNTY, WV -- At 15:08 on Wednesday July 1st, Station 11 (Bethany Pike VFD) and Station 30
(McKinleyville VFD) were alerted for a Possible Structure Fire on Rt. 67 in the area of Greens Run Road.
McKVFD Engine 36 (w/4-Lt. 304 Williams) and BPVFD Engine 110 hit the streets at 15:10. BCEMA 1 (Fowler)
arrived on scene at 15:11 and advised Brooke 911 of a working structure fire in a vacant 1-story dwelling
with heavy smoke showing from the eaves and windows. BPVFD E110 arrived on scene and advised the
incoming Engine 36 to hit the plug. Crews from E110 and E36 advanced a 1 ¾” cross lay off of E110 through
the front door and began fire attack. McKVFD R38, Sq3, BPVFD T119 and Sq111 then arrived on scene. In the
process, the interior crew came across numerous fuel cans in the storage room and advised command.
Crews then were in the process in placing a PPV at the front door when the house flashed over while crews
were still inside. The interior crew then advanced the line towards the seat of the fire located in the back of
the house and quickly placed the fire under control. At that time, Command requested that Station 20
(Bethany VFD) and Station 10 (Wellsburg VFD) to respond to the scene for manpower. Personal on scene
then began ventilation as BVFD E27 and WVFD E103 arrived on the scene. Crews then stretched 2 additional
1 ¾” cross lays from E110 and E36 along with a 2 ½” blitz line from E110. Crews then began a lengthy salvage

ST. KEYSER, WV -- On July 4th, The Potomac Fire Company's Engine Tanker 22 was alerted to s working
building fire. The 22 crew went to the 2nd floor of the building for fire attack and ventilation. The crew
encountered heavy gauge wire mesh covering all of the interior windows. Crew members worked to
removed the wire and then ventilate the windows on the 2nd floor. The crew came across fire in multiple
locations on the 2nd Floor. Fire was in the attic area, 2nd floor rooms, and between floors.

DENVER BOROUGH, PA -- Early on Saturday July 11th, the BOX Alarm was transmitted for the reported house
on fire in Denver Borough. Luckily, Denver Firefighter Matt Grant (Lives nearby) Just completed cleaning up
his prized Mack CF Engine for the Pump Primers event held in Harrisburg later that day. FF Grant quickly put
his wagon into service as the house next door was well off with multiple other homes as exposures. The
wagon pipe and one water line was put into service as units from the first alarm arrived. Crews worked for
about 2 hours to control the blaze which completely destroyed the 2.5 Story single family dwelling. Later that
day at the Pump Primer Competition, Matt's Mack won the the fastest Draft and pull contest.

EAST COCALICO TOWNSHIP, PA -- On Saturday July 11th, The Rescue Local was transmitted to the area of
Hahnstown Road and Glenwood Drive for the reported PIC with one trapped. Crews arrived with-in minutes
with the driver trapped in a four door vehicle after a collision with a pick-up truck. Rescue Squad 13 along
with Engine Company 17 had the extrication completed with-in 5 minutes. EMS units from the Reamstown
Volunteer Fire Company and Ephrata Hospital's Medic 4 transported the patient to a local trauma center.

PROVIDENCE TOWNSHIP, PA -- On Saturday July 18th, The Rescue Local was transmitted to the area of Route
272 & Mount Hope School Road for the reported PIC with people trapped. Units arrived on scene moments
after dispatch to find a two vehicle collision with one trapped. EMS and Fire crews worked to extricate the
driver in the PT Cruiser. While working EMS crews confirmed the Patient trapped succumbed to their
injuries. Two other patients involved were transported to local trauma center.

KALAMAZOO COUNTY, MI -- Around 4pm on Wed 7/15/09 firefighters from Comstock North FD were called to
respond to the area of East H Ave and Sprinkle Rd. for a MVA. R/E922 went into service and arrived a few
minutes later just north of the intersection and found 2 vehicles that had collided. Firefighters found that
there were no injuries in one vehicle and there was one injured in the other. Crews determined that the
drivers door needed to be removed so the cutters and spreaders were brought out and the door was
removed within minutes. The driver was removed by personnel and transported by Pridecare to the local

One Confined, Kalamazoo County, MI 07.15.2009

EASTWOOD, MI -- In the early afternoon hours on Wed. July 15th. Eastwood T/L842 hit the street along with
Pavilion Twp. FD and Comstock Central FD responding to an industrial fire at Green Bay Packaging in
Pavilion Twp. Firefighters arrived to find that a fire had started in the cardboard compactor inside the
building. T/L842's crew went to the roof to check for extension while PFD and CFD crews extinguished the
fire. Once the fire was knocked and there was no extension found all hands went to work in removing all the
wet cardboard from the compactor itself. Crews were onscene for several hours until the task was finished.

Industrial Fire, Kalamazoo County, PA 07.15.2009

HELLAM TOWNSHIP, PA -- At approx. 0552 hours, the rescue local assignment was transmitted for the vehicle
accident with people trapped on Route 30 in Hellam Township. Rescue Engine 41, Rescue 41-2 and Deputy
Chief 41 (Command 41) responded on the run with ten volunteers. Other units on the assignment included
Engine Company 21, EMS units, Lancaster Rescue 7-9, and Lancaster Engine 8-1. Deputy 41 arrived on scene
and found a two-vehicle collision with a total of 7 patients, some lying on the ground. There was no
entrapment. Volunteers from Wrightsville 41 arrived and immediately began patient care efforts, as Engine
Company 8-1 arrived and assisted with vehicle depowerment and provided a protection handline. All crews
on scene assisted with patient packaging and cleanup of debris from the roadway. Rescue Company 41
returned to service within an hour and a half.

EPHRATA TOWNSHIP, PA -- Shortly after 2200 hours on Tuesday July 28th, The street assignment was
transmitted to the south bound Route 222 off ramp to Route 322 for a reported tractor trailer on fire. Engine
Company 15 along with EMS from 112 were advised by Communications of a working car carrier on fire on the
off ramp, Chief 15 arrived moments later confirming the reports and requested Tanker 313 to the scene for
add'l water. Engine 15-2 placed two water lines in service as three vehicle burned on the trailer and was
spreading to add'l vehicles. The bulk of the fire was knocked with-in 5 minutes as the balance of the
assignment arrived. Later in the incident Rescue Squad 115 was added for add'l resources and manpower.

Car Carrier Fire, Lancaster County, PA 07.28.2009

CHEROKEE COUNTY, SC -- On July 28th, Blacksburg FD was responding mutual aid to a structure fire for a
barn struck by lightning,after severe thunder storms came thru the area. Squad 91 was driven by a 18 year
old firefighter when he drove thru some standing water in the roadway causing him to hydroplane and spin
off the road and then overturn. The driver was wearing his seatbelt, was transported to Upstate Carolina
Medical Ctr in Gaffney SC for evaluation and treatment of some minor cuts and scrapes. SC Highway Patrol is
investigating. CKC fire Department and Blacksburg handled the fire that burned a building to the ground but
saved a near by residence, and Buffalo FD responded to handle the overturned fire truck.
