WARWICK TOWNSHIP, PA -- At 3:41 AM the Twin Valley Fire Department along with Station 89 Ambulance
(Elverson EMS) and Medic 93 (Brandywine), were alerted to a motor vehicle accident with possible
entrapment in the 2000 block of Ridge Road in Warwick Township. Chief 69 (C.Witkowski) made the response,
and was advised by the Chester County Fireboard that they received reports of one vehicle into a pole with
the operator believed to still be inside. Ambulance 89-1 arrived and confirmed one vehicle through a
telephone pole, into the woods, with the occupant heavily trapped. Engine 69-1 and Rescue 69 arrived with a
crew of ten firefighters. Engine 69-1's crew led off with chainsaws, and began to clear a path to access the
vehicle. Rescue 69's crew provided some much needed lighting and began planning an extrication. The
patient was heavily trapped inside the passenger car, hanging upside down in thick brush and vines. An air
medical helicopter was requested to the scene to quickly transport the patient to a trauma center. With the
TVFD crew tied up on the extrication, Engine 62-1 (Ridge Fire Co.) was requested to establish a landing zone
at the Warwick Elementary School. TVFD apparatus cleared at 5:19 AM, while the TVFD fire police remained
on scene until 8:14 AM providing traffic control for the state police investigation and telephone company
repairs.

PIC w/Heavy Entrapment, Chester County, PA 09.03.2009
PEQUEA TOWNSHIP, PA -- On Sunday September 6th, The Rescue Local was transmitted to the area of Route
272 North at East Boehms Road for the reported PIC with people trapped. The West Willow Volunteer Fire
Company along with Rescue-Engine 55-1 and Engine & Rescue Squad 512 responded on the alarm along with
EMS from Providence Ambulance 59, LEMSA 612 and SVEMS 77. Crews arrived with a multiple vehicle
collision with heavy entrapment, crews freed the patients and transported them to LGH in Lancaster City.
Police report that the Buick LeSabre was traveling southbound in the northbound lane and struck Honda
Accord head-on. The Passenger in Buick died at LGH with the Driver of the Buick in critical condition. The
operator of the Honda recieved minor injuries from the collision.

Fatal PIC w/Heavy Entrapment, Lancaster County, PA 09.06.2009
BOROUGH OF STRASBURG, PA -- Early on the morning of September 9th, BOX Alarm 510 was transmitted to
123 East Main Street for the reported apartment building on fire in the Borough of Strasburg. Before units on
the first alarm responded and with multiple reports of people inside the structure, Lancaster County Fire
Radio added one add'l EMS unit to the BOX. Chief officers from Company 510 arrived on scene with-in min's
reporting fire showing from a 2.5 story woodframe OMD and requested the full second alarm with three add'l
tankers be added to the alarm. Crews worked the fire for about 1 hour before making a complete knock on all
the hot spots. One firefighter was taken to the Hospital after passing out while another refused treatment
after suffering minor burns.

Second Alarm w/Injuries, Lancaster County, PA 09.09.2009
SALISBURY TOWNSHIP, PA -- The Gap Fire Company was dispatched to 5197 Lincoln Hwy for a vehicle
accident involving a tractor trailer on September 9th. Chief Kurtz arrived on location reporting a vehicle on
it's side with 2 subjects trapped inside. The crew of Squad 42 stabilized the vehicle with rescue struts as the
crew of Rescue 42 removed the roof to gain access to the patients.

One Overturned After Police Incident, Lancaster County, PA 09.09.2009
ONTARIO COUNTY, NY -- Shortly before 6 a.m., September 10, Honeoye-Richmond firefighters were
dispatched to the Country Colony Estates, 5430 Curtis Road for a reported structure fire. Fire units arrived
on location and found a working fire in a eight-unit apartment building. Mutual aid was requested to the
scene from Springwater, Bristol, Hemlock, Livonia, East Bloomfield and West Bloomfield. Water was shuttled
to scene by tankers from a hydrant located on Route 20A about a mile from the fire. The fire was brought
under control in about 90 minutes. There were no injuries. Residents of the effected apartments were being
assisted by the American Red Cross. The cause was of the fire is under investigation by the Ontario County
Office of Emergency Management.

Working Apartment Building, Ontario County, NY 09.10.2009
EAST LAMPETER TOWNSHIP, PA -- Late On Saturday September
12th, The Rescue local was transmitted to the area of Old
Philadelphia Pike and Laurel Oak Lane for the reported PIC
involving a Departmental vehicle with multiple patients trapped.
Units on the ticket from Companies 63, 48 and EMS from LEMSA
were advised of a two vehicle crash involving Medic 100 with
patients trapped in both vehicles. Crews arrived reporting two
patients trapped, One being the Medic. Crews from
Rescue-Engine 63-1 and Rescue Squad 48 went to work to free
the patients as EMS provided rapid care to all involved. Injuries
were non-life threatening as both patients were taken to the
local trauma center for priority one check ups. PUMPFIRE and
it's staff wish a quick recovery to all invovled.

PIC w/Dual Entrapment Involving EMS, Lancaster County, PA 09.12.2009
WEST LAMPETER TOWNSHIP, PA -- On Wednesday September 16th, The Rescue Local was transmitted to the
area of Beaver Valley Pike and Long Lane for the reported PIC with two rolled over with people trapped.
Crews from Company 512, 54 along with EMS from 77 & 53 arrived on scene with a Chevy Blazer and Jeep
Cherokee both rolled over with one still inside the Jeep. Crews worked to free the driver as EMS provided
aid. Companies remained on scene for about one hour to assist Police.

PIC w/Dual Rollover & One Trapped, Lancaster County, PA 09.16.2009
FREEDOM TOWNSHIP, PA -- Station 40 (Freedom Township Fire Co.) & 60 (Friendship Fire Co.) was
dispatched for box alarm 40-1 for a vehicle accident with injuries at dunnings highway and Johnstown road,
Freedom Township. Blair county 911 stated that this was to be a head on collision involving a EMS unit that
was transporting a patient. Engine / Rescue 40-31 and QRS 40-41 responded on the call 40-01 and 40-82
responded direct on the call. While Station 60 was assembling a crew to respond 40-01 marked on scene and
he cancelled station 60. 40-31s crew was requested to set up the light tower and also set up for traffic
control 40-41s crew was also requested to set up for traffic control. Crews asst. EMS with patients care and
provided debris and fluids clean up. After the towing service arrived and removed the vehicles 40-01
released all Station 40 units. Engine / Rescue 40-31 and QRS 40-41 was back in service and available. 40-01
had command.

2nd Annual Lancaster City Fire Foundation Day, Lancaster County, PA 09.26.2009
WARWICK TOWNSHIP, PA -- On Saturday September 26th, Lancaster Fire Radio Transmitted the rescue local
to the 600 block of Millway Road in Warwick Township for the reported personal injury collision with people
trapped. Police Department arrived moments later reporting one trapped after a head-on collision with
serious injuries. Engine Company 210 along with EMS worked and aided patients as the Wagon crew started
extrication on the female trapped. Rescue Squad 115 arrived during operations to complete the extrication
of the patient from the dodge neon. Companies assisted PD with investigations as EMS transported the
patients to local trauma centers.

Head-on Collision w/One Trapped, Lancaster County, PA 09.26.2009
BRISTOL, WV -- At 15:21, Company 9 (Salem VFD), Engine 71 (Nutter Fort VFD), and Doddridge Engine 31
(Smithburg VFD) were alerted for a Structure Fire on Shaw’s Run Road just off of Hall’s Run Road in Bristol.
Engine 73 (w/6-DC Haddix) hit the street seconds later. SVFD Engine 91 arrived on the scene and advised
HC911 of a hay barn on fire with approximately 300-400 bales of hay inside fully involved. Engine 91 then
stretched a 1 ¾” cross lay and began the attack. Command 9 then advised incoming units that a diesel fuel
tank and storage shed was within 100 feet of the barn. Engine 73 arrived on scene as the 2nd due engine
and dropped the 3” supply line into E91 while 73’s packed crew walked up to the scene and stretched a 2nd
1 ¾” cross lay off of 91 and assisted with fire extinguishment, while Co. 9 protected the exposures. Mother
Nature then lent her helping hand in the efforts by allowing a down pour of rain to keep the fire contained to
the barn and allowing the firefighters to take a much needed rest but still protecting the exposures. With the
rain in no sign of slacking down Command 9 then held the box to Salem Fire units and released all mutual aid
units. Engine 73 operated for just over an hour before returning in station by 17:15.

400 Bales Of Hay Fuel Barn Fire, Harrison County, WV 09.20.2009
Marion, VA -- At 07:10 Hours on Saturday morning, Marion firefighters were dispatched to the 200 block of
Greenway Ave for a working house fire with possible entrapments. Ladder 1 arrived to find heavy smoke and
fire coming from side B of the residence. Tanker 2 arrived on-scene and established a water supply by
catching a near-by hydrant. Bystanders advised crews that all occupants had exited the structure prior to
our arrival. An interior attack was made into the house using a 200' 1 3/4 preconnect. A quick knockdown was
made and a PPV fan was set up to assist with ventilation. The interior was cleared using the TIC, and salvage
and overhaul were completed. No injuries were reported on the fire ground and all units cleared at 08:50.

Working House Fire, Smyth County, VA 10.10.2009
ONTARIO COUNTY, NY -- On Saturday afternoon, October 10, Richmond Fire and EMS were dispatched to a
rollover MVA on County Road 15. Fire and EMS units arrived on location and found a Time Warner Cable van
on it's roof in a ditch with the driver still in the vehicle. The male victim was flown Strong Memorial Hospital
in Rochester by a Mercy Flight Central. The Ontario County Sheriff's Office also responded and were
investigating the cause.

One Flown After Rollover PIC, Ontario County, NY 10.10.2009
CITY OF CHESTER, PA -- At around 10:00hrs Company 82, Engine 83, Medic 83 & Chester 11 were disp to 300
block of E. 11th st. for the building fire in the area. Company 82 went on scene to find a 2 story twin with
smoke showing. Engine Company 82 ran an 1 3/4 line to the 2nd floor to a bed room off. Truck 82 opened up
& Engine 82 stood by as the RIT. Companies were inservice for about 2 hours.

Heavy Smoke & Fire Showing On Arrival, Delaware County, PA 10.14.2009
LIVINGSTON COUNTY, NY -- On Saturday afternoon, October 17, the East Avon Fire Department and Avon
EMS/ALS responded to a two-car accident with entrapment in front of the East Avon fire station on West
Henrietta Road. Firefighters used hydraulic rescue tools to extricate the driver, who the the sole occupant,
from one of the involved vehicles. She was transported to Strong Memorial Hospital in Rochester. The
driver and passenger of the second vehicle were not injured. The Livingston County Sheriff's Office
responded to the scene and were investigating the cause.

EAST COCALICO TOWNSHIP, PA -- On Sunday October 18th, BOX Alarm 110 was transmitted to 73 Line Road for
the reported house on fire in East Cocalico Township. Police Department arrived reporting a working
building fire with multiple exposures. Chief 110 took the command on arrival and requested add'l manpower
from Company 13. Crews placed lines into service as E110-1 had it's own plug in front of the building.
Companies had the fire knocked and under control in about 25 minutes. One person was transported from
the scene by Ambulance 17 for minor burns.

Outbuilding Well Off On Arrival, Lancaster County, PA 10.18.2009
KENNETT TOWNSHIP, PA -- Station 25 was dispatched to Hillendale Road and Kennett Pike (Route 52) in
Kennett Township for a vehicle accident on Monday evening around 21:25 hours.  Units arrived to find a
pickup truck on its side with a 53 year old female driver trapped.  Pickup truck was proceeding thru a stop
sign on Hillendale Road and was struck by a BMW.  Driver of the BMW was uninjuried.  Kennett Fire Company
was added to the assignment to assist Longwood. Patient was extricated in 25 to 30 minutes and transported
to Christiana Hospital, Delaware by MICU 25-3.

One Overturned w/People Trapped, Chester County, PA 10.20.2009
Multiple Home Involved, Schuylkill County, PA 10.22.2009
MARION, VA --At 12:27 Hours On Tuesday, Marion firefighters were dispatched to the 45 mile marker south
bound on Interstate 81 for a PIC involving multiple vehicles with numerous injuries. Ladder 1 responded
within minutes and arrived to find a Budget Rental truck overturned with negative entrapments and two
other vehicles pulled to the side of the interstate. Marion Lifesaving Crew transported one patient to Smyth
County Community Hospital. Ladder 1 assisted in traffic control until Collins Towing and Recovery arrived.  
VA State Police and Marion Police Dept also assisted on-scene. Ladder 1 operated on-scene for about an
hour and returned to the station.

EAST DRUMORE TOWNSHIP, PA -- Quarryville Fire Company was dispatched to 100 Sunset Drive in East
Drumore Township for a dwelling fire on Tuesday afternoon at 16:18 hours by Lancaster County-Wide
Communications for a dwelling fire. Quarryville was assisted by Bart Township Fire Company and Rescue
5-12 from Willow Street for the RIT assignment.  EMS provided by SVEMS 7-7-9 and Ambulance 5-9
Providence Township. The cause of the fire at 100 Sunset Drive in East Drumore Township is still under
investigation by Pennsylvania State Police Fire Marshals. There was nobody home when the fire broke out in
an old farmhouse that was converted to two residences.

Working House Fire Displaces Seven, Lancaster County, PA 10.28.2009
KALAMAZOO COUNTY, MI -- On Tuesday October 27, 2009 around 0845hrs Kalamazoo Dept. of Public Safety
Dispatch began receiving several calls reporting that a small plane had crashed on the north end of the
Kalamazoo/Battle Creek International Airport near Kilgore Rd. The Box Alarm assignment was sent out which
brought Airport Crash/Rescue R2 and R3, Kalamazoo Dept. of Public Safety E2, E6, E7, T6. First arriving units
confirmed a single engine plane had crashed next to Kilgore Rd. and was on fire. A second alarm was struck
which brought City of Portage E2 , E3 and Comstock E911 and E912. Airport units quickly had the fire knocked
and the second alarm assignment was cancelled. The pilot had somehow missed a large building and several
cars in the parking lot before skidding to stop. The pilot did not survive the crash. The FAA and the NTSB are
investigating the incident.

Fatal Plane Crash, Kalamazoo County, MI 10.27.2009
BOROUGH OF NEW HOLLAND, PA -- Late On Thursday October 29th, Volunteers from the Liberty Volunteer
Fire Company and New Holland EMS along with Medic 9 were alerted for the reported PIC with people
trapped and the vehicle on fire in the 500 block of East Main Street. Units arrived on location reporting a
single vehicle crash with one trapped into a house and no fire involved. While crews started to extricate the
priority one patient, EMS requested aero-medical for the direct fly of the patient to LGH. Extrication was
completed in about 10 minutes with Skycare landing in Liberty's East Firehouse's parking lot. Only one
person was injured from the crash and remains under investigation.
